FIRST VISIT

Your first visit to a new dental office can be scary and confusing.  Our goal is to
reduce the anxiety of your first visit by helping you become familiar with the
process of your initial visit.  (If you require the use of a wheel chair ramp, please call in
advance and let us know; we will have it in place for your appointment.)
When you arrive, look for our entrance (pictured to the right).  When facing the
front building of the Farmington Boulevard Office Park, we are the last 2 suites on the right
hand side.

Our Appointment Coordinator will greet you with a smilie, assist your sign in and help you complete your New Patient Registration.   If you would like to get a head start, you can complete our New Patient Registration forms and bring them with you.

The majority of our forms are now electronic.  Please call ahead to have us email or text the custom link to your forms.  Once you receive the link, you will login to the forms using only your last name and date of birth.  Should you have any issues, please call us and we can provide assistance.  If you are not able to complete them using the link, we have a tablet that we can push the forms to in our office.  If you are needing to use the tablet to complete your forms, please arrive 15 -20 minutes early for your appointment.
